myPortledge Forms

Please be advised that all required forms for the 2023-2024 academic year (with the exception of the Physical Examination Form) are now available to be filled out online. Please log on to your myPortledge portal (linked here) to view the following forms:

  • myPortledge Profile Review
  • Emergency Contacts
  • Parent/Student Partnership Agreement
  • COPPA and Online Learning Consent (in the event online learning is necessary at any time)
  • G-Suite Parent Consent
  • Grandparent Information
  • Child Custody Form (please note that both parents are required to sign)
  • Permission to Charge (School Store), Photograph, and Publish 
Please log on to your myPortledge account before September 5th, 2023, to review and complete any outstanding forms. A detailed "how-to guide" is linked here and can help you navigate through this new process. 
 
As a reminder, the myPortledge portal is an essential resource that can be used throughout the year to view student schedules, access school directories (how-to guide linked here), read report cards, see student attendance records, view lunch menus, and more. We encourage new and returning users to log on as soon as possible to ensure there are no issues prior to the start of school. Please remember that in the event of an emergency closure due to inclement weather, we will use the information we have in myPortledge to send a text, email, and phone call. If this information is not correct, you will not receive the important message.
