Health Reminders

As we wrap up the school year and prepare for 2025–2026, please review these important health-related reminders. 
Magnus Health Portal: Annual Requirements
Portledge uses the Magnus Health app for secure and convenient access to your child’s medical records. All required health forms must be uploaded and completed in Magnus before the first day of school. We do not accept paper forms. **Please note that there may be different due dates for students participating in athletics. 
 
Required documents include:
  • Annual Physical
  • Up-to-date Immunization Records
  • Medication Permission Forms (signed by both physician and parent)
  • Action Plans (for asthma, allergies, diabetes, seizures—if applicable)
    • Food allergy and Anaphylaxis Emergency care plan forms must be filled out and signed by a physician and parents
  • Sports Health Update Form (SHUF) for all interscholastic athletes due by August 18 for JV and Varsity student-athletes and September 1 for Middle School student-athletes. 
  • Permission to Treat Form
All forms will be available in myPortledge on July 18. 
 
To access Magnus, log in to the myPortledge portal, click on Resources, select the Magnus Portal & Health Forms tile. Use a laptop or desktop with Google Chrome for the best experience. If you have questions or need help with Magnus, contact Magnus support at (877) 461-6831 or service@magnushealthportal.com. Required Vaccinations
Per New York State law, the following vaccinations are required before the start of the school year:
  • Tdap Vaccine for students entering 6th grade
  • Meningococcal Vaccine for students entering 7th grade
  • Second Meningococcal Vaccine for students entering 12th grade 
Please contact your child’s physician to ensure compliance by September 2025.
 
Other Health Office Notes
  • Emergency medication (inhalers, EpiPens, etc.) must be in the original container with a signed self-carry form.
  • During the year, grades K, 1, 3, 5, 7, and 11 will undergo hearing and vision screenings (unless you opt out).
  • All medications for overnight trips must also be in original packaging with updated permission forms.
Health Email Notifications 2025-2026
Starting in the 2025–2026 school year, Portledge will notify the community only when required by the New York State Sanitary Code, which mandates the reporting of suspected or confirmed communicable diseases.
